I don't think it's closed comedones, because if that was the case, even if you squeezed them, nothing would come out. It looks like a mix of bacterial and fungal acne (malassezia folliculitis).
It's time to stop with all actives! Go back to basics: cleanser, moisturizer and sunscreen. Let your skin (your sister's skin π
) heal and that might take some time. Introduce one product at the time. Let it work for 1 or 2 weeks. Then, introduce the other product. Discontinue if the irritation comes back.
Maybe your sister's skin can't tolerate both products. Sometimes the product is not to blame, but to give it a chance, she needs to go slow with the actives.
If the irritation persist, please search the help of a board certified dermatologist.

Omg I am so sorry for what happened to your sister. Better to stop using actives as of the moment and just stick to basics (cleanser-moistirizer-sunscreen).
Avoid harsh cleansers also as they can aggravate your irritated skin more. Your moisturizer is good because it has ceramides to help repair your sister's skin barrier. Wear sunscreen also everyday and avoid sun exposure as much as possible. Do this for at least 2-4 weeks and see the difference.
