Hello, the closed comedones may be caused by a product from your routine that's clogging the pores. Please revise other products from your routine or go to basics (cleanser, moisturizer and sunscreen) for a couple of weeks and see how your skin reacts.

I have actually gone back to basics. A non-comedogenic cleanser, the Benton BHA toner, and Innisfree Blueberry Moisturizer. Perhaps I just need to double cleanse.
Thank you for replying!

that’s strange that the derms only recommended extractions rather than also giving you something topical (I’m no expert tho!)
I can’t quite tell what the percentage of BHA in the cleanser is, but if it is low, you may want to give higher % BHA a try. The cosrx contains 7% AHA, which is quite normal for exfoliation.
Or if you don’t want to go down the exfoliation route, you may try benzoyl peroxide, azaleic acid, or retinoids (like adapalene). If it’s severe, perhaps you can ask for prescription strength retinoids (like tretinoin or isotretinoin)?
Here are some other factors that may contribute to acne/ closed comedones.
1. not enough hydration
Dehydrated skin compensates by producing more oils (which can lead to acne). If this is your case, try adding in a hydrating toner or essence.
2. too much moisturization
You could be using products that are too thick for your skin and clogging your pores, resulting in acne. Switch to a more lightweight/ less comedogenic moisturizer.
These other tips may help with your oiliness.
- oil cleansing/ double cleansing
- use a product with niacinamide (helps with oil regulation)

I am definitely hydrated. From what I've read, I think I really need to consider double cleansing, and if that doesn't work, I'd try a higher concentration since all of my BHA products have low concentration.
My dermatologist recommended me a topical gel, but she mentioned that it was to soften the comedones so she could extract it easier.
