Are you first time using active ingredients? I experienced the same thing back then because I never tried niacinamide and suddenly chose the 10% and I combined it with AHA toner. This experience made my skin breakout just like what's shown in that picture.
You should analyse what products you are using and try to search on the internet about do's and don'ts combining ingredients as some aren't meant to be combined.
